Submittal Instructions
Filling out the Request for Analysis will provide
all of the information required to process your
samples. Instructions for report and invoice
distribution should be included with each sample
shipment. For convenience and to improve turnaround
time, the Request for Analysis can be filled
out on-line or can be printed to accompany your
samples.
If submitting on-line, make sure to include date
of shipment, carrier of shipment (e.g. post,
FedEx,
DHL, etc.) and the waybill number so that we
can track any delayed shipments. An accurate
sample
list can be sent as an e-mail attachment to ancaster@actlabsint.com
and will speed up order processing. Poorly labeled
or packaged samples, or samples having incomplete
or no submission sheets may not be processed
until adequate written instructions are received
from
the client. Additional sorting charges will be
incurred.

Print a blank copy of the Request for Analysis
form to complete by hand and submit with samples.
If you need to ensure proper chain-of-custody
documentation, use our Chain-of-Custody Record
instead of the Request for Analysis and submit
the completed form with your samples.
